We are pleased to announce that AutumnCare's Interim Forms Release update is available now!
This update includes brand new clinical and operational forms, as well as enhancements to existing forms within the AutumnCare document set.
These updates have been developed based on emerging changes in clinical best practice, workflow enhancements and requests from our client base.
WHAT'S NEW?
Brand new forms include:
- End of Life Care
- A comprehensive clinical pathway for end of life care, associated with a Comfort Care Chart to accurately document symptom management
- Nutrition Management
- In conjunction with Nestle, AutumnCare now offers the EAT-10 swallowing screening tool and the Extended Mini Nutritional Assessment
- Delirium Screening Tool
- In partnership with Dementia Support Australia, AutumnCare now provides an electronic version of the Delirium Screening Tool.
- Suspected UTI Pathway With & Without Catheter
- Consent for Outings
- Prescriber Profile
WHAT'S UPDATED?
In addition to a suite of brand new forms, we've made improvements to existing forms, including:
- All tick boxes that generate a care plan will now become required fields to prompt the user to create a care plan
- Consent forms have been expanded to capture consent for more areas, including My Health Record and NDIS
- The Dietary Needs Assessment now shares information with the Fluid Management Directives form and the Fluid Balance Chart
- The Referral Form has been redesigned to include the outcome of the referral and the option to include the last recorded vital observations
