AutumnCare is pleased to announce our partnership with the Palliative Aged Care Outcomes Program (PACOP) to deliver a fully integrated, digital PACOP solution within the AutumnCare Clinical Care System.

This new development brings all PACOP assessment tools into a modern, intuitive interface—designed to support aged care teams in providing high-quality, person-centred palliative and end-of-life care. By embedding PACOP directly into everyday clinical workflows, AutumnCare reduces documentation burden, enhances data accuracy, and supports more timely clinical decision-making.

PACOP’s evidence-based outcome measures—focused on pain, symptom management, comfort, and quality of life—are now seamlessly accessible at the point of care. With real-time data capture, designed to align with PACOP reporting and benchmarking processes, providers can more easily demonstrate quality outcomes and drive continuous improvement.

System Requirements: To access the PACOP solution, services must be operating on AutumnCare Desktop 2024.2 and Forms 2025.1 (or later).
