Our system contains comprehensive clinical assessments designed to capture and communicate a high-level of detail surrounding:

Preset reports can be run to identify all current Risk Assessments and Dignity of Risk Forms within the system.
Identify clinical issues
Identify changes in a client's clinical status through notes, messages and automated alerts.
AutumnCare promotes the documentation of detailed, frequent progress notes and chart entries for each client. AutumnCare also contains alerts that will prompt staff to take action if a record falls outside of the predetermined parameters.
This allows staff to be informed as to clinical occurrences that may require further attention or preventative action e.g. BGL is falling outside of usual range, falls are occurring more regularly for a client, a PRN is being administered more frequently etc.
Manage infections and wounds
A system that is designed to put clinical matters front and centre is ideal for managing infections and wounds effectively.
Infections are documented within Infection Notification forms and communicated via automated messages to predetermined staff groups.
Wounds can be tracked via comprehensive wound assessments which allow for photos to be uploaded to demonstrate healing progress.
Both can be tracked live utilising AutumnCare's dashboard indicators, which keep a count of active wounds and infection incidents across the entire home care service.
Report on NQIP
AutumnCare contains inbuilt tools designed to assist providers to meet their National Quality Indicator Program (NQIP) reporting requirements.
The collection of data on the quality indicators within existing workflows ensures that additional workload is not being placed on staff.
